Who can refer a patient to the palliative care team?
Anyone can call the Palliative Care office at (352) 291-5881 for information or to make a referral. The office is open from 8-5, Monday-Friday.
The Palliative Care medical director and/or advanced practice nurse will meet with the patient first to determine the goals of care with the patient and family.
They confer with the patient’s physician and decide which other team members are needed and the optimal course of action.
